Hospital Liaison / Marketing Representative

PostAcute Care Partners seeks a Hospital Liaison / Marketing Representative to build and maintain strong referral relationships with hospitals, physicians, and care teams. In this role, you will educate clinical staff on our post-acute, rehabilitation, and skilled nursing services, coordinate smooth patient transitions from hospital to our facilities, and promote programs that support recovery, independence, and long-term wellness. You will analyze referral trends, conduct on-site hospital visits, present clinical program capabilities, and collaborate with interdisciplinary teams to drive census growth while ensuring a patient-centered, recovery-focused experience.
