Role Description The Licensed Practical Nurse (LPN) will provide direct nursing care to clients in their homes as part of an interdisciplinary care team. This part-time, on-site role is based in Alexandria, VA and involves performing routine nursing assessments, administering medications, monitoring vital signs, and assisting with treatments as directed by a registered nurse or physician. The LPN will educate clients and caregivers on care plans, disease management, and safe home care practices, while documenting all care accurately in accordance with agency policies and regulatory requirements.

Responsibilities include coordinating with other healthcare professionals, reporting changes in client conditions promptly, and supporting safety and infection control standards. The LPN will also contribute to care plan updates and participate in team meetings to support high-quality, client-centered home health services.

Qualifications

  • Active Licensed Practical Nurse (LPN) license in the state of Virginia and current Basic Life Support (BLS) or CPR certification.
  • At least one year of experience in home health nursing.
  • Demonstrated clinical skills in medication administration, vital sign monitoring, wound care, and routine nursing procedures in a home health or similar setting.
  • Strong assessment, documentation, and care coordination abilities, with attention to detail and regulatory compliance.
  • Effective communication and interpersonal skills for collaborating with clients, families, caregivers, and multidisciplinary team members.
  • Ability to work independently in client homes, manage time efficiently, and adapt to changing client needs and schedules.
  • Reliable transportation to travel to client homes in and around Alexandria, VA and willingness to adhere to infection control and safety protocols.
  • Prior experience in home health, geriatric care, or community-based nursing is preferred but not required.
