We don’t just treat symptoms—we restore hope, build sustainable resilience, and help people embrace a healthier future.Position OverviewThe Substance Use Disorder (SUD) Case Manager role is a 1099, contracted roll (not eligible for benefits), and provides comprehensive, recovery-oriented case management services to individuals with substance use disorders, including those with co-occurring mental health conditions. This role focuses on care coordination, resource linkage, advocacy, and ongoing support to promote treatment engagement, stability, and long-term recovery. Services are delivered in accordance with agency policies, Medicaid requirements, and applicable state and accreditation standards. The role is a remote, telehealth position Essential Duties and Responsibilities<ul><li style="list-style-type:disc;">Complete initial and ongoing assessments related to substance use, recovery needs, and social determinants of health.</li><li style="list-style-type:disc;">Collaborate with the interdisciplinary treatment team to develop and update individualized service plans.</li><li style="list-style-type:disc;">Coordinate services with therapists, medical providers, recovery residences, courts, probation/parole, and community partners.</li><li style="list-style-type:disc;">Link clients to housing, employment, transportation, benefits, medical care, and recovery support.</li><li style="list-style-type:disc;">Provide recovery-oriented, person-centered case management services.</li><li style="list-style-type:disc;">Complete timely, accurate, and compliant documentation in the electronic health record.</li><li style="list-style-type:disc;">Monitor client safety, relapse risk, and psychosocial stressors.</li><li style="list-style-type:disc;">Participate in supervision, team meetings, and required training.</li></ul>Qualifications<ul><li style="list-style-type:disc;">Associate’s degree required, Bachelor’s degree preferred</li><li style="list-style-type:disc;">Experience working with individuals with substance use disorders or co-occurring disorders preferred.</li><li style="list-style-type:disc;">Active CDCA Certification Required, LCDC II or comparable credential preferred.</li><li style="list-style-type:disc;">Strong communication, organization, and time-management skills.</li></ul>Work EnvironmentTelehealth, home office-based, and/or clinical setting. Reliable Internet is required for remote work. Local travel may be required, Community-based as needed.
